I have a couple questions please in desperate needs of answers.

It is said to provide evidence of my us citizenship, evidence that I visited and went to a hotel with my fiance, evidence that we are together.
I'm just wondering how do I provide these evidences like do I just print out of our pictures together, visits, our stay in a hotel, plane ticket, and evidence of my us citizenship? I'm very confused haha
my second question is,
is this where they want me to send the form after filing it completely?

Lastly, what if I forget or miss a step on part of the filing form and it gets rejected. Would they still take my money and have me send another one?
