Multiple last names in filling.

To hide identity I will use a fake name for my Brazilian fiancé but the same setup.

Let’s say the name is Isabella Cortez dos Santos.

Isabella is the first name.

Cortez is in her mothers family name from her grandmother. Her mother, uncle and everyone has the Cortez before the next name. So I don’t think it’s a middle name.

dos Santos is defiantly her last name because it is a common name in Portuguese.

My question is when filling out forms for visa which option below is correct?

Option one: First Name - Isabella Middle Name - Cortez Last Name - dos Santos

Option two: First Name - Isabella Middle Name - N/A Last Name - Cortez dos Santos
