A PHD in Norway or Canadian PR?

This is a genuine doubt of mine and i am confused now. I was pretty clear about Canada but the healthcare system is irking me a lot. You have to wait months to get a dermatology consultation; that's seriously messed up. This got me thinking is it really worth it.

One of my friend is in Europe and he seems pretty happy with Europe and Norway seems to have a good scope for PhD so I am now confused😡😡

Background: I am 29 years old, got 5 years of work ex in marketing analytics and I have a double masters : MBA in international business and marketing ; PGP in Data science and business analytics.

Please provide some guidance πŸ™πŸ™πŸ™

Note: I am gay btw so I need to take that into considerationπŸ˜…
