Questions about visa extension and travel

My friend has a visa which she extended twice Both applications are in process and pending reply from immigration;she is worried about traveling back to India due to the COVID situation there

My concerns that I can’t find an answer for online (and we aren’t able to get in touch with them by phone ) are 1- does the extension apply on after the date of expiry of the original visa ? Or from the date of application!

2- can she safely apply to extend for a 3rd time due to the COVID situation India and her health concerns (vaccinated , but considered otherwise at higher risk ; alsoUS currently is recommending not to travel to India due to the situation there) She hasnt heard back anything on the previous extensions

3 - are there any places where there is a guaranteed turnaround time for COVID testing required for plane travel

Any help is appreciated I am very concerned and she is anxious as we haven’t been able to get in touch with anybody and find this info
