If a CBP agent recommends a visa for next visit, what does this actually mean?

Between late 2019 and Sep 2020 I had to enter the US four times on an ESTA for medical treatment following complications after laser eye surgery. I'd previously been living in the US as an E3 visa holder, and had the surgery in the US. Each ESTA visit was less than 90 days, but on my last visit I was sent for a Secondary. I submitted a FOI request and the agent has written "Subject advised to obtain a visa for next trip."

Just curious - does this mean that my next ESTA application will automatically be rejected? Will I still need a visa even if I wish to enter the US as a tourist?

NVC Case FE Review note

Has anyone else received this message? All of our documents say "Approved" on NVC. Does this mean we have to find another sponsor? Can we show the consular officer our
