Embassy Requested passport

Does anyone know if its good sign that embassy has requested a passport for submission?
My mother had her interview 3 months ago on June 16, she was placed in AP. Colombia embassy has requested her passport for submission last month in August.

The following are her status updates:
June 16-refused.
July 30-ready.
August 10-Administrative process.

August 23-She received email from embassy to submit her passport.
August 26-She got a call from a case manager at embassy asking if she submitted passport already which she obviously already did. Minutes after that phone call her status changed to refused again.

Were still waiting.

Any experience that can be shared is appreciated.

A few questions: I filed my N400 naturalization on March 23, 2020. Nearly one year later on Feb 2021 they sent me a notice that they'll reuse my biometrics from my green card application, but they won't refund the biometric fee!
