US citizen returning to US with a legal permanent resident with dementia

I will be traveling internationally in December with my father who has dementia. He needs help as he sometimes gets confused. I am a US citizen while he is still a permanent resident. At immigration (while returning), he and I will have goto different counters as the counters for US citizens and permanent residents are not same. What can I do so that we are not separated in such situation?
