stem opt first evaluation range of dates after switch job?

Hi,

I switch the job 1 month after stem opt started.

and sevp showing I need to submit first evaluation with current employer.

so what should be my range of this evaluation dates. should I put start date as stem opt start date or new employment start date. and end date should be 12 months after the start of stem opt minus one day?
