Approved visa - received passport with F1 form stapled within?

I had my visa approved last week, and received the expected package today.

All it contained was my passport with the F1 form I had given them during the interview, folded and stapled inside of the passport.

Is this normal? Is there anything else I was supposed to receive?

Also not sure if I’m supposed to take the form out or leave it stapled inside?

I’m a bit confused. I thought I would have received some sort of official document proving I have a valid F1 visa.
