Birth certificate name doesn't match current passport name

Hello, The name on my birth certificate, C Fern, was changed to S Fern in a matter of a few months after my birth. Me and my fiancé are currently applying for the K1 visa and we are in the process of compiling documents for the I-129F packet.

I was wondering if a sworn affidavit attested by my country's consulate is sufficient in this case to prove both names belong to me.

Just wondering if this is okay as we are doing everything by ourselves. Thanks!
