After applying for an I-485, what should you do with H-1B extensions?

Suppose an H-1B holder applies for an I-485 in November, but by December he's supposed to start applying for the H-1B extension petition, should the employer be doing that? i.e. both processes?

It looks like it may take around 14 months post I-485 for the employee to receive a Green Card. So, if he travels outside the USA after H-1B extension is approved and before Green Card arrives, should he still get the H-1B visa stamped?
